Vintage on the Ponds is a privately owned, 70-bed licensed community-based residential facility or CBRF. The Vintage is situated on 116 acres amidst wildlife, trees and flowers overlooking two large ponds. We have three outdoor courtyards for our residents to enjoy with family and friends. All rooms are private, however, they vary in size, décor and bathroom arrangement.
We offer 24-hour nursing supervision and a qualified support staff. Our staff give excellent care and take pride in Vintage residents giving them a comfortable place to live and call “home!” The level of care provided ranges from residents who are independent with all activities of daily living to those residents who require total assistance in all areas. Dignity and respect are of utmost importance for those who live here and our goal is to always provide the highest quality of care and support to our residents resulting in the best quality of life!
We have a wonderful activity staff that keep residents involved and busy with numerous activities daily. We have a small, wheelchair accessible bus to accommodate those who enjoy going out on outings and we provide transportation to and from doctor appointments as well. Vintage on the Ponds provides respite, hospice and palliative care. We are also contracted with Family Care.

VINTAGE ON THE PONDS
RESPITE CARE
The Vintage offers respite care for individuals whose dedicated caregivers need a break to rest and rejuvenate. This could give the caregiver a day, weekend, week, or up to 28 days, for personal time. Do you care for a loved one and need a day or two to do things for yourself? Does your situation require you return to work? Let us help by providing respite care, so it is possible for you to enjoy some free time and regain the energy and vitality needed to care lovingly and adequately for your loved one. The rates for a respite stay are provided, for your convenience, in this folder on our rate sheet. Respite care is provided on a first come, first served basis and when room availability allows.
We are also able to provide respite care for individuals in need of rehabilitation services. Vintage on the Ponds is a comfortable, home-like environment unlike some hospital or institutional settings. We are equipped to provide the same intense therapy as other facilities, such as skilled nursing homes, while you enjoy the country setting, beautiful grounds and privacy of your own room. Information about our on-site rehabilitation services is also included in this folder.

HOSPICE CARE
Hospice services are provided, at The Vintage through several highly regarded companies. When a loved one is in need of 24-hour palliative or end-of-life care, our staff of professionals will partner with the Hospice group of your choice to assist. Your loved one is able to remain in the privacy of their own room while receiving the collaborative care. Family can relax knowing that our nurses are available to your loved one when needed. Hospice care is available to those who reside at the Vintage or who are guests on a limited respite stay. Hospice staff also provides support services to the family including medical, social, and religious. The Hospice teams and our staff have been working together for years. We strive to ensure the final road of one’s life is comfortable with dignity and respect. Families are welcome day and night, and are encouraged to stay with their loved one. We can also accommodate those who are visiting from out of town, at a reasonable room rate.

TRANSPORTATION OPTIONS
The Vintage offers handicapped accessible van transportation for our residents to and from appointments. Please contact us for rates.
Delavan Rescue Squad Medical Transport offers van and ambulance transportation services in our area. They are providing professional transportation to and from medical appointments with a qualified Emergency Medical Technician (EMT) assisting and driving. The van is equipped with a lift that will accommodate residents utilizing a wheelchair.

Activities & Events
“My mother has been at Vintage on The Ponds since February 28th. My sister works as a chef in that facility. Mother is there for both respite and rehab. They have a rehab facility right on the grounds, so there is no need for her to be taken to and from rehab. She can just go down the hall from her room. Right now, they are doing just light rehab. So far the care has been wonderful. The people there are very, very caring. It is a very small private facility. If you walk up and down the hall, no matter what time of the day, you will see people checking in rooms and seeing how the patients are. If they want to discuss something or if they need someone to stay and talk with them for a few minutes, they are very willing to do that.”
“My mom has lived here about a month, and we are very happy with the facility and the staff. Everyone has been very kind, helpful and accommodating. Mom has her own room, which is small but perfect size for her. She shares a bathroom with a neighboring room, which is empty now, so she has it to herself for now. Vintage has a very cozy atmosphere, very home-like, with a library, an activity room (called “parrot room” because there is a parrot in a cage) and comfy decor. There are several pianos, which the residents play, as well as guests. They keep mom very busy, which was a challenge at her last assisted living facility. Here she has exercise class, bingo and a lot of entertainment. The food is really good. Best of all, the staff members seem happy to be there and are very attentive to her needs”
